Ongoing Home Care

Regular help from a caregiver who knows them

When the same caregiver visits on a regular schedule, they learn your parent's routines, pace, and habits. That makes it easier to notice small changes early and act before they become larger concerns

Our approach to home care

Each visit builds on the last

Every CareQuilt client has a matched caregiver, an RPN care manager who reviews every visit, and monitoring between visits. Over time, your parent's care team learns what works for them, and care becomes more personal. Before care starts, we take time to learn three things.

Connected home care

Care you can count on, day after day

Your care team reviews every visit and follows up on changes over the weeks. Your care manager is your point of contact, and an RPN oversees their care. When needs change, the plan changes with them, without starting over.

Tell us what youโ€™ve been noticing

You don't need to know what kind of care to ask for. Tell us what's changing, what's becoming harder, or where a little more support could help. We'll figure out what makes sense together.
